Denise M. Fales

Denise M. Fales, 72, of Meadville, passed away on September 29, 2016, in Phoenix, AZ with her son and daughter in-law by her side.

Denise was born in Perth Amboy, NJ, the daughter of the late Michael and Jean Markulin of Meadville. Denise was married to Dana R. Fales for 25 years; he preceded her in death in 1992.

A resident of Crawford County for many years, she had previously resided in New Jersey as well as Chicago and Pittsburgh.  Denise was employed by the state of Pennsylvania for over 25 years.

Denise is survived by two daughters, Debra Fales-Gralewski of Meadville, Donna Fales of San Diego, CA and one son Derik Fales of Phoenix, AZ; six grandchildren Austin, Tyler, Parker and Cecilia Fales of Phoenix, AZ and Nicholas and Noah Gralewski of Meadville; a sister, Linda Markulin-Jenkins and her husband Merle Suelzie of Bremerton, WA; a daughter in-law Monica Fales of Phoenix, AZ; two son in-laws, Alan Gralewski of Meadville, and Hichem Mertil of San Diego, CA; two nieces, Jessica Czereszka and Cassie Redstone of Seattle, WA.

A funeral service will be held on Friday, October 14 at 10 a.m. at Stephen P. Mizner Funeral Home & Cremation Services, Inc., 404 Chestnut St., Meadville.

Interment will immediately follow at Evergreen Cemetery in Harmonsburg.

In lieu of flowers, the family requests that memorial contributions be made to American Cancer Society, 320 Bilmar Drive, Pittsburgh, PA 15205-4620.

The family would like to express a heartfelt thank you to the staff of Phoenix Mountain Nursing Center and the Ryan House (Hospice of the Valley) in Phoenix, for their care during Denise’s last days.
